Just a suggestion, I think giving them blindness would make it super spooky for them (if that's too limiting set fog to as close as possible), with a texture pack that adds ambience with scary music (disc 11 for vanilla scary sounds).

i think it would be more interesting if some loot could spawn in the backrooms, maybe chests containing small amounts of food and resources could have been the focus rather than the monster

i tried this, and these backrooms are too easy to escape. all you have to do is break one of the ceiling lights, mine the walls, craft planks with the spruce logs, and tower up.

Simple idea, yet so satysfying. I really hope to see this idea evolving into teams trying to exit with many monsters of different kind, different ways to defeat/stop/confuse them and marking walls with like drawings, a bit pvp, some goals and actuall possibility to win. Also could there be low chance of generating staircase up or down that leads to next level?
