I think it's because of what happened in Vietnam. Watch the episode Joel's Diary. You can see when Saya cuts his hand off- he screams like it really hurt. Poor guys been impaled, had limbs torn off, etc. several times, but this really hurts him because it was Saya that did it (his true love).
